Comments:
A Subsidy Health and Safety Inspection was conducted on March 28, 2019 review supplemental health and safety requirements. Licensing inspector arrived at 6:02am and departed at 7:45am to conduct a Subsidy Health and Safety Inspection. There were seven children present at arrival to the facility with three staff members. The children were observed arriving to the program, playing with various tabletop activities and talking freely amongst themselves. The licensing inspector reviewed the physical plant, supervision, required postings, children's records, emergency evacuation and shelter-in-place documentation, and nutrition. The posted menu indicated that today's snack would consist of yogurt and bananas. Three medications were reviewed. Five children's records were reviewed during the inspection. The exit interview was conducted with the Site Director. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on review, the vendor did not ensure that there was a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present. Evidence: A child's record was reviewed and it was indicated that the child had allergies and/or sensitivities that were not included on the center's allergy list.

Plan of Correction: We will go through the file and make sure everyone is on the list and that all staff know about each child's allergies. We will update the list.

Description: Based on review, the vendor did not ensure that all staff who work with children were informed of children's allergies, sensitivities, and dietary restrictions. Evidence: A child's record was reviewed and it was documented that the child had allergies and/or sensitivities that the staff were not aware of.

Plan of Correction: We will go through the file and make sure everyone is on the list and that all staff know about each child's allergies.
