Comments:
An unannounced monitoring inspection was conducted on April 9, 2019 during the aftercare portion of the program. The Licensing Inspector arrived at 3:05pm and departed at 8:00pm. Upon arrival to the facility there were no children present with two staff members. Throughout the course of the inspection there were three staff members and 17 children. The licensing inspector observed the children transitioning to the program, having snack, and playing at various tabletop stations to include but not limited to creative arts, puzzles and games, science and homework and talking freely amongst themselves. Five children's records were reviewed. The licensing inspector observed the physical plant, emergency and non-emergency supplies and documentation, nutrition, required postings, supervision, and health and safety. The exit interview was conducted with the Area Manager.

Violations:
Standard #: 22VAC40-185-260-A
Description: Based on review, the licensee did not provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: During the inspection conducted on April 9, 2019, the most fire inspection report for the facility was more than a year old and was dated 4/6/17.

Plan of Correction: Area Manager will contact the Fire Marshall's (state and local) to see if we can get our own inspection.

Standard #: 22VAC40-185-510-J
Description: Based on observation and inspection of the facility, the licensee did not ensure that medication shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection conducted on April 9, 2019, a prescription and a nonprescription medication were observed in the first aid kit stored on an open shelf behind a curtain in an area that was accessible to the children in care.

Plan of Correction: Area Manager will have a staff member to place it in a locked file cabinet on the stage.

Standard #: 22VAC40-185-540-E
Description: Based on observation and inspection of the facility, the licensee did not have all required nonmedical emergency supplies at the center. Evidence: During the inspection conducted on April 9, 2019, the battery operated flashlight was inoperable.

Plan of Correction: We will purchase batteries and have them in place by tomorrow.

Standard #: 22VAC40-185-550-D
Description: Based on review, the licensee did not ensure that the center would implement a monthly practice evacuation drill and a a minimum of two shelter-in-place practice drills per year for the most likely to occur scenarios. Evidence: During the inspection conducted on April 9, 2019, there was no documentation of an evacuation drill in March 2019 and one of two shelter in place drills in 2018.

Plan of Correction: We will have a completed fire drill and shelter in place tomorrow.

Standard #: 22VAC40-185-560-F
Description: Based on observation and inspection of the facility, the licensee did not ensure that when centers choose to provide meals or snacks all requirements are followed. Evidence: During the inspection conducted on April 9, 2019, the menu for the current one week period was not posted. The menu for the month of March was posted.

Plan of Correction: The menu will be posted tomorrow.
