Comments:
An unannounced monitoring inspection was completed on April 4, 2019 from approximately 11:00a.m. to 1:15p.m. Eighteen children were in care during the inspection. Staff and children, ages infant to four to years old, were observed in four classrooms with adequate staff ratios. The school aged classroom was also observed but the school aged children had not arrived yet. Children and staff were observed engaging in activities to include free play, preparation for nap time, nap time and coloring. Children?s equipment, learning materials, indoor space, rest rooms, the center's van and the first aid and emergency supplies were inspected. The outdoor space is not currently used. Five children?s records, five staff records, annual fire and health inspections, fire drill, shelter in place and lock down drill logs, daily health observation certifications, injury reports, daily attendance, the emergency preparedness plan and required postings were reviewed. The Director was available for interview and the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Standard #: 22VAC40-185-160-C
Description: Based on a review of staff records, the licensee failed to ensure that staff obtain and submit results of a follow-up tuberculosis screening a least every two years for one of five staff. Evidence: 1. Staff #1's record, hired on April 3, 2017, contained a tuberculosis test/screening dated March 22, 2017 and no subsequent test/screening as required. 3. Administrator #1 acknowledged the follow-up screening was not completed timely.

Plan of Correction: The Owner/Director will have the staff obtain a TB test/screening.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children?s records, the licensee failed to maintain all the elements of a child?s record for two of five children. Evidence: 1. Child #2's record, enrolled on November 12, 2018, and Child #3's record, enrolled on January 15, 2019 lacked documentation of chronic problems and accommodations and allergies and actions to be taken. 2. Administrator #1 acknowledged this information was not documented in these records as required.

Plan of Correction: The Owner/Director will have the parents complete this information.
