Comments:
The licensing inspector conducted a subsidy health and safety inspection Thursday, April 18, 2019 from 01:20pm to 4:00pm to review supplemental health and safety requirements. The children were observed resting on cots. They later had snack and played outside on the playground. All classrooms and the playground were inspected today. Five children?s records and five employee records were reviewed during this inspection. Medication is being administered and was reviewed. Last emergency drill: 03/18/2019 Last shelter-in-place drill: 03/19/2019 Total enrollment today: 25 Today, the following child to staff ratios were observed: Twos and Threes ?15:1(rest time ratio) Pre-K ?10:1(rest time ratio) School Age ?not present The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-540-C
Description: Based on observation on 04/18/2019, the center did not ensure that each required first aid kit contained the minimum items. EVIDENCE: 1. The first aid kit for Bus #1 did not contain a pair of scissors and two triangle bandages. 2. The first aid kit for Bus #2 did not contain a thermometer and one triangle bandage.

Plan of Correction: Will order or pick up the missing items by Tuesday, 4/23/2019.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on a review of staff records on 04/18/2019, the center did not ensure that staff obtained a fingerprint based national background check in the required time frame. EVIDENCE: The records of Staff #4 (date of hire 05/08/2017) and Staff #5(date of hire 12/04/2017) did not contain the letter of eligibility for the fingerprint based national background check results.

Plan of Correction: When staff return to work will have them make appointment here at the center for that day. Will make a copy of the appointment confirmation and put in the staff record.
