Comments:
An unannounced renewal inspection was completed on 04/22/2019 from 10:10 A.M. until 3:30 P.M. At the time of the inspection 41 children were in care with seven staff, and two administrator present. Five children's records, five staff records, four board members records, required posted information, evacuation drills, four injury reports, emergency plan, playground safety plan, fire inspection, health inspection, first aid kits and supplies, and outside play area were reviewed. Children were observed during free play with activities that included dramatic play, drawing and building with blocks. Children were also observed during a large group music and movement activity, handwashing, diapering, outside play, lunchtime, infant playtime, and naptime. Standard #: 22VAC40-185-160-C
Description: Based on a review of staff files, the center failed to ensure that a tuberculosis screening was updated at least every two years from the date of the initial screening or testing. Evidence: 1. Staff #2's last tuberculosis screening on file was dated 03/03/2017. 2. Staff verified that the last tuberculosis screening on file was dated 03/03/2017.

Plan of Correction: The staff member has an appointment this week to obtain an updated tuberculosis screening. In the future we will track when tuberculosis screening are due for updates and notify the staff in a timely manner.

Standard #: 22VAC40-185-260-A
Description: Based on a review of the fire inspection, the center failed to have an annual fire inspection completed. Evidence: 1. The fire inspection on file was dated 04/20/2018. 2. Staff verified that the most current fire inspection on file was dated 04/20/2018.

Plan of Correction: Administration placed a call to the Fire Marshall's office to get scheduled for an inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to maintain all equipment in a safe and operable condition. Evidence: 1. In the Two Year Old Classroom the middle cabinet door under the counter area is broken and when opened falls off the hinges. 2. Staff verified that the cabinet was broken and would fall off the hinges when opened.

Plan of Correction: Administration will contact maintenance and have the cabinet door repaired.

Standard #: 22VAC40-185-450-D
Description: Based on observation, the center failed to ensure that pillows were covered with pillow cases. Evidence: 1. In the Three/Four Year Old Classroom there was one pillow that did not have a pillow case. 2. In the Four/Five Year Old Classroom there were two pillows that did not have a pillow case. 3. Staff verified that the pillows did not have pillow cases covering the pillow.

Plan of Correction: Staff will talk with the parents tonight to obtain a pillow case for the pillows. In the future if the pillow has no pillow case it will not be used during naptime.
