Comments:
An unannounced monitoring visit and facility tour was conducted from 11:00am-1:15pm. At the time of inspectors' arrival there were 51 preschool aged children in care with 7 staff members and 2 administrators. Children were observed participating in arrival, indoor and outdoor play, toileting and handwashing, and transitioning between activities. A sample of 6 children's records and 6 staff records were reviewed. Medications and injury reports were also reviewed. Areas of noncompliance are noted on the Violation Notice and were discussed with the director at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record review and interview it was determined the center did not ensure that staff resubmit TB test results at least every two years from the date of the first initial screening. Evidence: 1. The record for staff #1did not have updated TB screening or testing documentation. The documentation available was dated 4/28/2016. 2. Director confirmed that updated documentation was not available.

Plan of Correction: Staff #1 will get updated TB screening or testing prior to returning to the center.

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. In the Red classroom(1 year olds) there was: -one metal corner on the bulletin board on the wall was bent outwards creating a 1/4" rough protrusion -the metal frame around the right side of the mirror was separated from the wall and mirror -there were 19 standard sized staples on the floor by the bulletin board and carpet 2. The mechanical/electrical room door was open leaving screws, nails, tools, and aersol duster accessible to children in care 3. In the Keva around the windows there was: -peeling paint -peeling drywall -chipped/broken plaster -small holes (1/4")

Plan of Correction: A maintenance request will be submitted. Maintenance staff will be reminded to ensure the mechanical/electrical room door remains closed and locked.

Standard #: 22VAC40-185-510-N
Description: Based on medication review it was determined the center did not dispose of medication not picked up by the parent after 14 days of being notified about the medication authorization expiring. Evidence: 1. The medication authorization for child #7 expired 12/2018 and medication was still present at the center. 2. The Director confirmed that the medication authorization was expired and they did not have an updated authorization available.

Plan of Correction: Director will return the medication to the parent until a new authorization is provided.
