Comments:
A Subsidy Health and Safety inspection was conducted on 05/06/2019 to review supplemental health and safety requirements. There were 53 children, ages 5 years-11 years, and 6 staff members present during the inspection. The inspector reviewed 5 children's records and 1 medication during the on-site inspection. The on-site requirements for 6 staff members were reviewed during the inspection; the full staff records will be reviewed at a later date at the Corporate Office. The children were observed in the following activities: during afternoon snack, participating in large motor play in the gym and during homework time. The inspector reviewed the emergency preparedness plans and the emergency supplies during the inspection. The inspector discussed the license renewal process and subsidy requirements with the staff. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and can be found on the Violation Notice. Description: Based on observations, review of attendance records and discussion with the staff, the vendor failed to ensure that for each group of children a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: There was not a system place that recorded the departure of children as it occurs. Arrival of children is noted, however, departure is not.

Plan of Correction: A system was created to document arrival and departure.
