A Subsidy Health and Safety inspection was conducted on May 13, 2019 to review supplemental health and safety requirements. New subsidy requirements became effective on October 17, 2018. The on-site inspection was from 10:50 a.m. to 2:40 p.m. On this date, there were 25 children present in five classrooms and in the care of five staff members. There were two additional staff members available to assist if needed. There were 37 children enrolled in the center. All five classrooms were toured and two of those classrooms were observed. Five staff members' records and five children's records were reviewed. Seven staff members and three children were interviewed. The medication and medication paperwork were reviewed for one child. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Standard #: 22VAC40-665-540-A Description: Based on review of five staff records and confirmation from staff, two new staff members failed to obtain a tuberculosis (TB) screening before their date of hire. Evidence: 1. The record for Staff #4 contained a TB screening dated 04/06/2019 and the date of hire for Staff #4 was 04/02/2019. 2. The record for Staff #5 contained a TB screening dated 03/09/2019 and the date of hire for Staff #5 was 03/07/2019. 3. Staff verified that the two staff members did not obtain their TB screenings before their date of hire. Plan of Correction: Staff is not aware of the new regulation and from now on will ensure staff have their TB screenings before their hire date.
