Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted on May 13, 2019. The on-site inspection was held from 10:50a.m. to 2:40p.m. On this date, there were 25 children present in five classrooms and in the care of five staff members. There was one additional staff member available to assist, if needed. All five classrooms were toured and observations were made in two of those rooms. There were 37 children enrolled in the center. Five staff members' records and five children's records were reviewed. Seven staff members and three children were interviewed. The medication and medication paperwork were reviewed for one child. The lunch served met the USDA guidelines and consisted of spaghetti with meat sauce, crackers, peas, mandarin oranges, and milk. The exception to compliance was noted on the violation notice and the risk rating was noted on the supplemental page. The fire marshal had been contacted to request the annual fire inspection. Some items reviewed/observed: required posted information, warm water temperature, documentation of evacuation and shelter-in-place drills, first aid kit and emergency supplies, choice time, music and movement, story time, infant care, diaper changing procedures, hand washing and bathroom procedures, lunch, and nap time. Violations:
Standard #: 22VAC40-185-290-3
Description: Based on observation and confirmation with staff, one electrical outlet did not contain a protective cover. Evidence: 1. One electrical outlet in Classroom D did not contain a protective cover, posing a hazard to children in care. 2. Staff #2 verified that a cover was missing from the outlet.

Plan of Correction: Immediately during the inspection, a cover was placed into the outlet.
