Comments:
An unannounced Monitoring inspection was conducted during the after school program. The LI arrived to the program at approximately 2:45PM. Reportedly, 15 children are currently enrolled in the program. There were 12 children and two staff present. Staff reported that the ages of the children ranged from 5 years to 10 years. The children and staff were observed on the outdoor playground, during afternoon snack, homework time and free play. A sample of 6 children's records was selected for review. Documentation of the annual fire and health inspections was reviewed. Medications were observed in the center. The inspection concluded at approximately 5:30PM. Standard #: 22VAC40-185-60-A
Description: Based on staff interview and a sample review of children's records, the Center failed to ensure a separate record for each enrolled child was kept at the center and information as required by these Standards was complete in each child's record. Evidence: 1. When requested by the Licensing Inspector, staff reported that records for two children (children 5 and 6) were not available for review in the center. 2. The following information was incomplete in reviewed children's records: 60.A.5.) Name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached; 60.A.7.) Allergies and intolerance to food, medication or any other substances, and action to take in an emergency situation. 1. Two of four records did not contain a complete physical address for each listed emergency contact (child #3, a PO Box given) and child 4). 2. Allergy information was not addressed for child 1.

Plan of Correction: All children's records will be maintained on-site. Staff will review each record with the child's parent to ensure complete information is on file for each child.

Standard #: 22VAC40-185-70-A
Description: The Center failed to maintain the following information on site for each staff: name, address and phone number of a person to be notified in an emergency. Evidence: 1. Emergency contact information was not available in the center for staff #2. 2. Staff reported they were serving as a volunteer, the individual was assigned as the second present staff member in the center.

Plan of Correction: Information will be maintained in the center for all staff.

Standard #: 22VAC40-185-510-G
Description: Based on medications observed in the program, the center failed to ensure all medications were dispensed from a pharmacy and maintained in the original, labeled container. Evidence: The prescribed medication for child a. was not maintained in the original container.

Plan of Correction: The Teacher will inform the child's parent and will ensure medication is only accepted when brought into the center in the original container.
