You are reminded that one additional shelter in place is required prior to the end of the school year.

Comments:
An unannounced monitoring inspection was conducted this date. A total of 43 children with 3 staff present. When the inspector arrived, the children had transitioned from outside to inside and were eating their daily snack. Once snack was completed, children broke into small groups where they completed homework, played with electronics and painted with water colors. All medications were reviewed today. A total of 7 children's records reviewed and staff CPR/First Aid, Daily Health training and medication training were verified. Staff records are reviewed annually at the administrative office. Observation of pick up by parents where staff gave some a daily update on their child. Equipment such as balls to play in the gym, books, table top activities such as legos and dominoes were available for the children to independently select. Standard #: 22VAC40-185-50-A
Description: Based on observation of the programming area, confidential information pertaining to children was accessible to the general public. Evidence: Repeated from the November inspection, the confidential list of children's name remains accessible to the general public. A list of which children have allergies is also posted and accessible.

Plan of Correction: We will take the information down immediately.

Standard #: 22VAC40-185-60-A
Description: Based on review of children's records, 2 out of 7 records reviewed were not complete. Evidence: Child #1 and #2's record did not contain the health exam and documentation of immunizations.

Plan of Correction: We will have the administrative office scan both into their electronic files.

Standard #: 22VAC40-185-510-D
Description: Based on review of all medications on site, two medication authorizations expired. Evidence The medication authorization for Child #4 for both Auvi Q and Benadryl expired in March 2019.

Plan of Correction: We will obtain a new authorization from the physician.

Standard #: 22VAC40-185-510-G
Description: Based on review of all medications, written authorization for two medications were not obtained. Evidence: The authorizations for Benadryl and Auvi Q for Child #3 is for Loudoun County Public Schools and not Loudoun County Parks and Recreation.

Plan of Correction: We will have the physician complete the correct authorization form.
