A non-mandated monitoring inspection due to a self-reported incident received by the Licensing Office on 05/10/2019 was conducted on 05/10/2019 and 05/23/2019; the allegation was related to a forbidden action. During the 05/10/2019 inspection, video footage was viewed, 2 staff records and 1 child's record was reviewed and 2 interviews with staff took place. An third staff member was interview at the 05/23/2019 inspection. There were also representatives from other agencies on-site during 05/10/2019 inspection. There were no violations issued as a result of the self-reported incident. The inspector arrived on 05/10/2019 at 10:20 a.m. and departed at 2:30 p.m. and arrived on 05/23/2019 at 1:20 p.m. and departed at 2:00 p.m. For questions, please contact Becky Forestier at 540-309-2835.
