Technical Assistance:
Licensing inspector discussed child abuse and neglect training with human resources manager.

Comments:
An unannounced monitoring inspection was conducted today from approximately 12:30pm-4:30pm. There were 144 children in the direct care and supervision of 40 staff. A sample of 10 children?s records and 7 staff records were reviewed. The children were observed napping, eating snack, reading and getting ready for dismissal. Two new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the administrator. Standard #: 22VAC40-185-160-C
Description: Based on record review, staff did not resubmit TB test results at least every two years from the initial screening. Evidence: 1. Staff A did not resubmit TB results at least every two years from the first initial screening. 2. The TB screening of Staff A was last complete 05/08/2017.

Plan of Correction: Staff will be asked to get updated test results tomorrow.

Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records were incomplete. Evidence: 4 of 10 children's records reviewed were missing required information. 1. Child A was missing the address of one emergency contact and immunization records. 2. Child B was missing the phone number of both parent's employer. 3. Child C was missing the employer and employer phone number of both parents. 4. Child D was missing the address of one emergency contact.

Plan of Correction: Parents will be asked for required information.
