Comments:
An unannounced monitoring inspection was conducted on May 31, 2019 from approximately 2:00 pm-4:00pm. Observations included programs, administration, supervision, special care provisions and emergencies, health and safety, and staff and children's records. Five staff records and five children's records were reviewed. The School Age children were observed arriving to the center from other schools and programs and talking freely in the teen room. There were 4 children present with two staff members. Additional staff were on site for administrative and support purposes. Exit interview was conducted with the Childcare Director.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review, in one of five staff records reviewed, the licensee did not ensure that each staff member submit documentation of a negative tuberculosis (TB) screening. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering. Evidence: The TB screening submitted for Staff #1 was dated 6/29/17 which was more than 12 months prior to the staff member's hire date of 3/7/19. This was confirmed by Staff #1.

Plan of Correction: Center director will have the staff member get a TB screening within a week.

Standard #: 22VAC40-185-60-A
Description: Based on record review, in two of five children's records reviewed, the licensee did not ensure that each child's record was maintained with all required information. Evidence: As confirmed by Staff #1, the records for Child #1 and Child #4 did not have the work phone numbers for both parents documented. The record for Child #5 did not have the work phone number for one parent.

Plan of Correction: The record for Child #1 was corrected by the parent during the inspection. The center director contacted the parents of Child #4 and Child #5 for correction.
