The inspector conducted an unannounced mandated inspection from 11:00-until 2:00. There were 60 infants and children in care. Infants were observed, playing, being fed, and resting. Children were observed playing inside and outside, washing hands for lunch, eating lunch, and resting. No mediations are being administered at this time. One violation had been corrected and one violation had not been corrected from the last inspection. Seven children's records were reviewed and and six staff records were reviewed. Standard #: 22VAC40-185-60-A Description: The center failed to maintain all the required information for children's records. Evidence: 1. The inspector reviewed seven children's records. Three out of seven records did not all the required information. Record #1 and #2 did not have info. on allergies or chronic illnesses completed. Record #3 did not have emergency addresses listed. Plan of Correction: Director will ask parents for the information and check the other files for completeness.
Standard #: 22VAC40-185-330-B Description: Based on observation, the center did not maintain resilent surfacing under all equipment. Evidence 1. The inspector observed a piece of playground equipment (a bus with a slide) that was not over resilient surfacing, it was over the concrete. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-185-350-E-1 Description: Based on interview and observation, the center did not maintain ratios. Evidence: 1. The director stated that due to family emergencies and vacations, the infant room was not in ratio. 2. The inspector observed 5 infants and one staff person in the infant room for the majority of time in the infant room. The director assisted in the room when not busy with other duties. Plan of Correction: Due to two family emergencies and vacations, the center was short staffed today. The rest of the week more staff is available and scheduled to work so that ratios will be met.
Standard #: 22VAC40-185-350-E-2 Description: Based on observation, the center did not maintain ratio in the toddler room. Evidence: 1. The inspector observed ten children in the toddler room with two staff members; the youngest child's bday is 4/13/18 (child #7). Plan of Correction: Director will see that ratios are met.
Standard #: 22VAC40-185-560-F Description: Based on record review and interview, the center was not following USDA guidelines. Evidence: 1. The inspector reviewed a menu where water was listed with one other food element for snack. Plan of Correction: USDA guidelines were reviewed with the director.
