Comments:
An unannounced monitoring inspection was completed on 05/24/2019 from 9:30 A.M.-11:45 A.M., and on 06/03/2019 from 10:00 A.M.-2:00 P.M. At the time of the inspection 128 children were in care with eighteen staff and one administrator present. Ten children records, thirteen staff records, seven medications and required documentation, seven injury reports, fire inspection, health inspection, emergency plan, first aid kits, emergency supplies, playground, and posted required information were reviewed. Children were observed during infant feeding, one-on-one infant interactions, outdoor gross motor playtime, art activities, fingerplays, story time, handwashing, transition times, free choice activities that included building, dramatic play, sensory play, lunchtime, and naptime. Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that all hazardous materials were kept in a locked location.

Evidence:
1. In a Pre-School classroom there was a can of Lysol Disinfect Spray in a bin on the toy shelve that was accessible to children.
2. The warning label stated "keep out of reach of children, caution, and flammable."
3. Staff verified that the Lysol Disinfect Spray was not in a locked location.

Plan of Correction: Staff immediately moved the Lysol Disinfectant Spray to a locked location. Administration will review the hazardous material requirements with all staff.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the center failed to ensure that all electrical outlets had protective covers. Evidence: 1. In the big Pre-School Classroom there were two electrical outlets that did not have protective covers. 2. Staff verified that there were two electrical outlets without protective covers.

Plan of Correction: Staff immediately placed protective covers into the electrical outlets. Administration will review with all staff to complete a walk-thru of their classroom each morning to ensure all electrical outlets have protective covers.

Description: Based on observation, the center failed to ensure that all prepared infant formula bottles were dated and labeled with the child's name. Evidence: 1. In the Toddler Classroom there was a bottle in the refrigerator that was not dated and labeled with the child's name. 2. Staff verified that the bottle was not dated or labeled with the child's name.

Plan of Correction: Staff immediately dated and labeled the bottle with the child's name.
