Comments:
A Subsidy Health and Safety inspection was conducted on 7/8/2019 to review supplemental health and safety requirements. An unannounced renewal inspection was completed on 7/8/19. There were 46 children present. Five children's records and six staff records were reviewed. No medications were being administered at the center. The inspector arrived at the center at 10:20 am and departed at 5:30pm. The children were observed during various classroom activities, during movie times, during lunch, and during nap. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-510-B
Description: Based on review of documentation and interview with staff, the center failed to ensure that a written record of attendance was maintained for arrivals and departures of each child as is occurred. Evidence: 1. The children were counted while in the gym and the attendance records were reviewed. The inspector counted two children more than were marked present on the attendance. This was at approximately 11:15 am. A staff person identified two children who had arrived at approximately 9:00 am and 10:00 am who had not been marked present on the attendance. 2. The attendance for the school-age children was observed at approximately 2:30 pm. There were 14 children present. There were 13 children marked present on the attendance. The staff person stated that the child not marked on the attendance arrived at approximately 8:00 am. 3. The attendance for the infants was observed at approximately 2:40 pm. There were 10 children present in the infant room during the entire inspection. Seven children were marked present as of 2:40 pm.

Plan of Correction: This was corrected during the inspection. A staff person in each group marked the children present who had not been signed in on the attendance.

Standard #: 22VAC40-665-580-E-1
Description: Based on record review and interview, the center failed to ensure that all staff had current certification in cardiopulmonary resuscitation (CPR). Evidence: 1. Staff #1, #3, #6, and #7 did not have certification in CPR. All of the staff had completed a CPR training course July 2019 with practice demonstration as verified by certificates from a "Friends and Family" course but this course stated that it was not a certifying program or did not offer program completion certificates. 2. The director was interviewed and verified the above information.

Plan of Correction: All staff will complete the certifications.

Standard #: 22VAC40-665-580-E-2
Description: Based on record review and interview, the center failed to ensure that all staff had current certification in cardiopulmonary resuscitation (CPR). Evidence: 1. Staff #1, #3, #6, and #7 did not have certification in first aid. The director verified that these staff did not have first aid certification or training.

Plan of Correction: All staff will complete the certifications.
