Standard #: 63.2(17)-1720.1-B-3 Description: Based on record review, the center did not obtain an out-of-state central registry (CPS) background check for one employee who lived outside of the state of Virginia within the last 5 years. Evidence: Staff #1's (date of hire: 5/17/17) sworn disclosure statement confirmed that they lived in Arizona within the last 5 years. There is no documentation of an Arizona CPS background check being completed. Plan of Correction: 1. The staff person is no longer employed effective 7.26.19 2. To make copies of the documents and follow-up with the agency for out of state background checks. 3. All staff was given a copy of the policy and guidelines for Guidance, Protection of and Respect for Children. Staff will be re-trained on August 22nd.
