Thank you for your time and assistance with the unannounced renewal inspection conducted today, August 1, 2019. The on-site inspection was from 11:15 a.m. to 3:50 p.m. On this date, at the time of arrival, there were 49 children present in five classrooms and in the care of eight staff members. There were three additional staff members on-site to assist if needed. The total enrollment for the program was 90 children. The exception to compliance was noted on the violation notice and the risk rating for the violation was documented on the supplemental page. There were five staff members' records and five children's records reviewed. The medication and medication paperwork were reviewed for three children. Five classrooms were toured and observations were made in three of those rooms. Seven staff members and four children were interviewed. Some items observed/reviewed included: infant care and feeding, diaper changing procedures, nap time, choice time and free play, outdoor play, lunch, art activities and painting, parent authorizations for sunscreen, injury reports, evacuation, lock down and shelter-in-place drills, current health and fire inspections, and warm water temperature. Standard #: 22VAC40-185-130-A Description: Based on review of five children's records and verification from the owner, one child's record did not contain documentation that the child had received up-to-date immunizations per the age of the child. Evidence: 1. The record of Child #1did not contain a record of the immunizations the child had received. First date of attendance for Child #1 was 06/10/2019. 2. The owner of the center verified the immunization record had not been received. Plan of Correction: The parent of the child was called during the inspection and the center will obtain the immunization record as soon as possible.
