Comments:
An unannounced renewal inspection was conducted today from 8:15am-3pm. A total of 35 children were present with 7 staff. Children were observed during morning snack, lunchtime, napping, handwashing, music play, and engaging in free play. A sample of 5 children's records, 5 staff records, medications, and other documents were reviewed. Areas of non-compliance are documented on the violation notice. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on children's record review, each child did not have a physical examination within one month after attendance. Evidence: Child 1 (date of first attendance 07/01/2019) did not have documentation of a physical examination.

Plan of Correction: I will follow up with the parent to ensure the physical is in the record.

Standard #: 22VAC40-185-40-I
Description: Based on record review and staff interview, the center did not develop written procedures for injury prevention. Evidence: Staff acknowledged the center has not developed a written procedure for injury prevention.

Plan of Correction: I will type procedures.

Standard #: 22VAC40-185-40-K
Description: Based on record review and staff interview, the center does not have a written playground safety procedure that includes positioning of staff in strategic locations and methods of maintaining resilient surface. Evidence: Playground safety procedures provided by staff did not include positioning of staff in strategic locations and methods of maintaining resilient surface.

Plan of Correction: I will type up the procedures.

Standard #: 22VAC40-185-60-A
Description: Based on children's record review, the center did not maintain a complete record for each child. Evidence: 1. Child 2 (date of first attendance 6/25/19) record did not contain name, address, and phone number of a 2nd emergency contact. 2. Child 2's record did not have documentation of proof of identity.

Plan of Correction: I will request the information from the parent and update the record.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center inside were not maintained in a clean and safe condition. Evidence: 1. One tile located in the girl's restroom in the first stall closest to the entrance was cracked. 2. Eight tiles located in the boy's restroom outside of the two stalls furthest from the entrance were cracked. 3. Rust was observed at the bottom of the stall wall nearest the toilet in the first stall of the boy's restroom. 4. Multiple tiles beneath and around the crib wheels located in the infant room were discolored black. 5. The following play equipment were found with holes which create a finger entrapment hazard: Tool bench located in 4year old classroom, play sink in the 2 year old classroom, play kitchen set in the 3year old class.

Plan of Correction: (1,2,4) The tile floors are scheduled to be renovated. (3) I will sand down and put down rust proof paint. (5) We will replace the toys or fill the holes.

Standard #: 22VAC40-185-330-B
Description: Based on observation, playground equipment fall zones were not free of hazardous obstacles. Evidence: Playground apparatus used to spin while sitting was located within one foot of the bottom of the slide chute in the toddler area .

Plan of Correction: We will have maintenance move the equipment.

Standard #: 22VAC40-185-440-L
Description: Based on observation, pillows were used by children under two years of age. Evidence: Two children in the infant classroom were observed sleeping in the cribs with a boppy pillow under their heads.

Plan of Correction: Staff immediately removed boppy pillows and reported the pillows would be sent home with the parent.

Standard #: 22VAC40-185-500-B
Description: Based on observation and staff interviews, the diapering area was not provided with a sink with running water or a storage system for disposable diapers that is foot operated. Evidence: 1. The toddler and 2 year old classrooms did not have access to sinks with running water in the diapering area. Staff reported utilizing sink located in the hallway for handwashing which required opening classroom door to exit into the hallway. 2. The trash can located in the 2 year old classroom for diapering was not foot operated.

Plan of Correction: We will get the sinks running. We will replace the trashcan.

Standard #: 22VAC40-185-570-E
Description: Based on observation, bottles were not dated and labeled with the child's name. Evidence: One bottle located in the older infant's classroom was not dated or labeled with the child's name.

Plan of Correction: We will reiterate with the teacher to date and label all bottles.
