Comments:
The inspector conducted an unannounced mandated inspection from 10:30 a.m. until 1:00. There were 13 infants and children in care with staff supervising at all times. Infants were observed being fed, having a diaper change, and playing. Children were observed playing at outside, eating lunch, during rest time, and during diapering. Five children?s records and five staff records were reviewed. Areas of non-compliance are identified on the violation notice. Please contact your inspector at 276-698-9981 with any questions.

Description: Based on record review, the center did not have a physical examination in all the child's file. Evidence: 1. The inspector reviewed five records. One of five records (file # 4) did not have a physical examination on file and the child

Plan of Correction: The director says the physical is on file with the TAP program and will get a copy.

Standard #: 22VAC40-185-160-C
Description: Based on record review, the center did not have a repeat TB test every 2 years for a staff person. Evidence: 1. The center reviewed five child records. One of five (record # 2) did not have a repeat TB test, the last one was completed 03/20/2017.

Plan of Correction: The staff person will get the repeat TB screening or test this week.
