Comments:
An unannounced monitoring inspection was conducted today from 10:00 am until 1:45 pm. There were 89 children present with 16 direct supervision staff. The children were observed playing with table toys, coloring, toileting, hand washing and eating lunch. Lunch consisted of Chicken cubes with tomato sauce, baked potatoes, cauliflower, fruit and milk. The entire physical plant, children's records, staff records, attendance logs, evacuation logs, emergency plans, menu's and medications were inspected/ Areas of non compliance were cited on the Violation Notice. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, the center did not ensure that children's records are complete and up to date.

Evidence:
1) Child #1 and child #7's records
did not include documentation of addresses for both emergency contacts.
2) Child #3's record did not include documentation of address and phone number for the second emergency contact.
3) Child #5's record did not include documentation of an address for the first emergency contact.
4) Child #3's record does not include the name and date of person reviewing proof of identity.

Plan of Correction: We will get information from parents and place in children's records.

Standard #: 22VAC40-185-270-A
Description: Based on observation of the outdoor play space, the center failed to ensure that equipment if maintained in a clean, safe and operable condition.

Evidence:
1) On the back section of the fence, there are several nails that are protruding and could entangle clothing or snag skin.
2) The knob is missing from the green gear on the large play structure.

Plan of Correction: We will repair the fence and replace the knob.

Standard #: 22VAC40-185-290-3
Description: Based on classroom observation, the center failed to ensure that all electrical outlets will have protective covers.

Evidence:
The Purple classroom had 2 electrical outlets without protective covers and the Orange classroom had one electrical outlet without a protective cover.

Plan of Correction: We will replace covers.

Standard #: 22VAC40-185-510-C
Description: Based on a review of medications, the center failed to ensure that all center procedures were followed to prevent use of outdated medications.

Evidence:
Child #1's emergency medication expired 01/2019.

Plan of Correction: We will get updated medication from parents.

Standard #: 22VAC40-185-550-C
Description: Based on an inspection of the facility, the center failed to ensure that emergency evacuation procedures will be posted in a location conspicuous to staff and children on each floor of each building.

Evidence:
The center did not have the Emergency Evacuation Procedure posted on each floor of each building.

Plan of Correction: We will post.
