Comments:
The licensing inspector conducted an unannounced monitoring inspection from 9:20 am - 11:45 am. There were thirty preschool children in care: fifteen children with two staff in classroom #1 and fifteen children with two staff in classroom #2. The children were observed playing in the centers, in small group activities, at story time and preparing for lunch. Five children's records were reviewed. Violations from the previous inspection were also reviewed and all had been corrected. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-185-60-A
Description: Based on record reviews, the center failed to obtain all required information for the children's records.
Evidence:
1. The inspector reviewed five children's records. One of five records reviewed (child record #4) listed a parent as not authorized to pick up child; however, there was no legal paperwork in the file.

Plan of Correction: The teacher will contact the child's parent and request this information for the child's record.

Standard #: 22VAC40-185-330-B
Description: Based on observation, the center failed to maintain the resilient surfacing on the playground at the required depth of at least 6 inches.
Evidence:
1. The inspector observed the playground and measured the resilient surfacing (pea gravel). There was approximately 2 to 3 inches of resilient surfacing surrounding the mats and in the fall zones around the climber. The fall height of the climber is approximately 44 inches.

Plan of Correction: The teacher will complete a work order for additional pea gravels to be added in these areas.
