Comments:
The inspector conducted an unannounced mandated inspection from 10:15 a.m. until 11:45 a.m. There were 29 children in care with staff supervising at all times. The director and cook were also present. Children were observed reading stories in circle time and singing, during lunch, and washing hands. Infants were observed being read to, and playing. Medications and the corresponding paperwork were reviewed. Five children's records and six staff records were reviewed. An acknowledgement form was signed by the director. Description: The center failed to maintain the required depth of resilient surfacing on the playground.
Evidence:
1. The inspector observed mulch that was 2-4 inches around the slide and the swings.

Plan of Correction: The director will have staff/volunteers redistribute mulch around the slide and swings.
