Comments:
An unannounced monitoring inspection was conducted today from 10:00am-12:15pm. There were 50 children (infants-5yrs.) supervised by 16 direct care staff. The physical plant, 4 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed painting, exploring different sensory items in the sensory table, playing a counting apples game, and engaged in a variety of other activities. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, 3 of 5 classrooms were not maintained in a clean, safe, and operable condition.

Evidence:
1. In the Penguins classroom (children ages 3 years old), a ceiling tile was missing, creating a hole in the ceiling.
2. In the Pandas classroom (children ages 8 weeks through 2 years old), the crank handles to the windows were broken.
3. In the Puffins classroom (children ages 3 years old), there were a few small areas of chipped paint on the green wall.

Plan of Correction: We will work with the church to remedy the items.

Standard #: 22VAC40-185-550-D
Description: Based on record review, the center did not maintain documentation of a monthly practice of evacuation drills.

Evidence: There was no documentation of completed fire drills for April, May, and June 2019.

Plan of Correction: I will schedule them in advance, and do a drill without the bells, as needed.

Standard #: 22VAC40-185-560-G
Description: Based on observation, 1 of 10 lunch bags brought from home was not dated.

Evidence: In the 4-5 year old classroom, one child's lunch bag was not labeled with the current date.

Plan of Correction: Director will remind parents to date lunches while waiting for class to open. Staff will double-check lunches as children come in.
