Comments:
An unannounced monitoring inspection was completed on October 24, 2019 from approximately 1:30 p.m. to 3:30 p.m. Twenty-five children were in care and approximately six staff were on premises during the inspection. Staff and children were observed in classrooms engaged in activities to include activities for infants, nap time, and snack time. Interviews and interactions were conducted and no medications are given. Five children?s records and five staff records were reviewed. The owner/director was available for the inspection and present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations:
Standard #: 22VAC40-185-160-C
Description: Based on a review of staff records, the licensee failed to ensure that staff obtain and submit results of a follow-up tuberculosis screening a least every two years for two of five staff.
Evidence: 1. Staff #3's record contained negative tuberculosis screenings/tests dated August 8, 2016 and August 23, 2018. Staff #4's record contained negative tuberculosis screenings/tests dated August 23, 2017 and August 28, 2019. 2. Administrator #1 acknowledged these follow-up screenings were not completed within two years as required.

Plan of Correction: The director/owner will continue to remind staff of the two year requirement.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children?s records, the licensee failed to maintain all the elements of a child?s record for one of five children.
Evidence: 1. Child #4's record, enrolled July 22, 2019, lacked two emergency contacts. 2. Administrator #1 acknowledged this information was not documented and reports that the family indicated not knowing anyone to list as they recently moved to the area.

Plan of Correction: The owner/director will advise the parents today that they need to provide two emergency contacts other than themselves.

Standard #: 22VAC40-185-280-B
Description: Based on observation of classroom space, the licensee failed to keep hazardous materials locked using a locking method that prevents access by children.
Evidence: 1. Two bottles of spray disinfectant, a bottle of spray deodorizer, and a spray bottle of window cleaner were observed in three classrooms, unlocked and on open shelves. 2. Administrator #1 acknowledged these hazardous materials were not locked.

Plan of Correction: These items were locked on site and therefore this was corrected.
