Comments:
The inspector conducted an unannounced mandated inspection from 10:15 a.m. until 12:15. The 11 children in attendance were on a filed trip to the library and out to lunch. Five children's records were reviewed and staff records were reviewed at the main office. One medication is available for a child and it was on the filed trip. The violations are identified on the violation notice. Please contact your inspector at 276-698-9981 with any questions.

Standard #: 22VAC40-185-130-A
Description: The center did not obtain immunization documentation before children started.
Evidence:
1. The inspector reviewed five children's records. Two of five records (#1, #2) did not have immunization documentation.

Plan of Correction: Staff will get the documentation from the parents.

Standard #: 22VAC40-185-140-A
Description: Based on record review, the center did not obtain a physical exam within 30 days of enrollment.
Evidence:
1. The inspector reviewed five children's records. One of five records (#1, date of enrollment 8/13/19) ) did not contain a physical examination.

Plan of Correction: Staff will get the physical exam from the parents.
