Comments:
The inspector conducted an unannounced mandated inspection from 10:15 until 12:15. There were 13 children in care with staff supervising at all times. Children were observed playing in the gym, at circle time and at centers. One medication was reviewed and the corresponding paperwork. Five children's records and staff records were reviewed. Pl;ease contact your inspector at 27698-9981 with any questions.

Standard #: 22VAC40-185-130-A
Description: Based on record review, the center failed to obtain documentation for immunizations before a child attended the center.
Evidence:
1. The inspector reviewed five records. One of five records (#5) did not have an immunization on file.

Plan of Correction: Center staff will get the immunization update from the parents.

Standard #: 22VAC40-185-140-A
Description: Based on record review, the center failed to get a physical examination for each child.
Evidence:
1. The inspector reviewed five children's records. One of five records (#5) did not have a physical exam on file and the child had been enrolled longer than 30 days.

Plan of Correction: Staff will get the physical exam from parents.
