An announced inspection was conducted at the administrative building in Farmville on November 12, 2019 to review staff records. Eleven staff records were reviewed. If you have any questions, please contact the licensing inspector at (804) 662-9038.

Standard #: 22VAC40-185-160-A Description: Based on record review, the center did not ensure that two of eleven staff records contained documentation of a tuberculosis screening no later than 21 days after employment.
Evidence:
1. The record staff #2 (start date: 2/1819) had documentation of a tuberculosis screening dated 8/1/2019.
2. The record staff #11 (start date: 10/2218) had documentation of a tuberculosis screening dated 5/31/2019.Plan of Correction: Reviewed on a regular basis to ensure records are up to date.
