Comments:
An unannounced monitoring inspection was conducted today from 10:20am-12:15pm. There were 35 children (5-12yrs.) supervised by 3 direct care staff. The physical plant, 2 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in various board games, playing air hockey, Legos and with ipads, and caring for the programs' pet geicos. Hand-washing procedures were also observed. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-185-60-A
Description: Based on record review, 2 of 4 children's records did not contain documentation of all required information.

Evidence:
1. Child #1's (start date: 11/1/2019) record did not contain documentation of dad's place of employment, either parents' work telephone number, and addresses for either of the two emergency contact persons.
2. Child #2's (start date: 8/26/2019) record did not contain documentation of dad's work telephone number.

Plan of Correction: I will follow-up with the parents this evening.

Standard #: 22VAC40-185-280-B
Description: Based on observation, 2 of 2 classrooms did not ensure that all hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In the SACC 1 classroom, an unlocked cabinet above the sink contained a spray bottle of chemical cleaning solution.
2. In the SACC 2 classroom, an unlocked cabinet underneath the sink contained a container of WD-40 and a gallon of paint.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the program did not ensure that the findings of the most recent inspection of the facility were posted.

Evidence: The Violation Notice for the most recent inspection conducted on 5/16/2019 was not posted. The Summary page from the most recent Subsidy inspection was posted.

Plan of Correction: The Violation Notice will be posted today.
