Comments:
An unannounced monitoring inspection was conducted today from 9:30am-1:30pm. There were 39 children (infants-4yrs.) directly supervised by 8 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, being bottle fed, napping, coloring, playing pretend in the housekeeping area, and eating lunch. Today's lunch consisted of Sun-Butter, cheese, whole-grain flour tortilla, spinach, carrots, apple slices, and milk. There was an adequate supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice.

Standard #: 22VAC40-185-130-B
Description: Based on record review, the center did not obtain documentation of additional immunizations, once every 6 months for children under the age of two years.

Evidence:
Child#4's record contains documentation of immunization records that were last updated on 11/20/2018.

Plan of Correction: We will request a new immunization record, we have already talked to the parent.

Standard #: 22VAC40-185-140-A
Description: Based on record review, the center did not obtain documentation of a physical examination within 30 days of a child's first day of attendance.

Evidence:
Child#4's record does not contain a physical examination form. The child's first day of attendance was 1/8/2019.

Plan of Correction: We will request a physical examination today.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center, were not maintained in a safe and operable condition.

Evidence:
In the after-school classroom peeling paint was observed on the window sill by the block area, and two small holes (approximately 1 inch long) were also observed on the wall under the window.

Plan of Correction: We have already submitted a work order, it will be done before the end of the month.

Standard #: 22VAC40-185-510-G
Description: Based on review of medication and medication procedures, long term medication that was administered to a child was not maintained in the original, labeled container.

Evidence:
Prescription medication that was administered to Child#1 was not kept in the original container with a prescription labeled attached.

Plan of Correction: We will call the parents to bring the original container.
