Comments:
A Subsidy Health and Safety Inspection was conducted on 11/04/2019 from 8:45 A.M.-4:00 P.M. to review supplemental health and safety requirements. At the time of the inspection 24 children were in the care with seven staff and two administrators present. Five children records, seven staff records, four board members background checks, four injury reports, one medication and required documentation, first aid kit and emergency supplies, emergency plan, playground safety plan, evacuation drills, fire inspection, health inspection, outdoor play area, parent handbook, teacher handbook, and required posted documents were reviewed. Children were observed in free play activities that included dramatic play, drawing, and building with blocks. Children were also observed during a large group music/movement activity, outside play, handwashing, diapering, lunchtime, and naptime. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements tat are not covered in licensing standards are reviewed during the supplemental inspection. The exceptions to compliance are noted on the violation notice.

Description: Based on review of medications and children files, the center failed to obtain a written allergy care plan for each child with a diagnosed food allergy.

Evidence:
1. Child 1M had an emergency medication on site with an allergy to food products, but there was not an allergy plan on file for the child.
2. Staff verified that there was not an allergy plan on site for child 1M.

Plan of Correction: Administration will contact the parent and request a written allergy plan from the physician.
