63.2(17) License & Registration Procedures

Technical Assistance:
The next anticipated inspection will be the center?s renewal inspection. Center management was reminded to have all renewal application documents completed and sent into licensing office 45 days prior to expiration for processing. The renewal application is available on the VDSS website.

Background checks for one or more staff members were either missing or incomplete during today?s inspection. Licensee must provide documentation to the Licensing Inspector that the missing/incomplete background checks have been sent out within 10 calendar days of today's inspection. Failure to do so could potentially result in enforcement action.

Comments:
Today an unannounced Monitoring Inspection took place between the hours of 11:20am and 3:15pm. At the start of the inspection there were a total of 52 children with 10 staff; all ratios were incompliance. A total of 5 children?s files and 6 staff files were reviewed. The children were observed eating lunch, transitioning and napping. Seven medications and authorizations were reviewed. Interactions between staff and children were positive in nature. Violations:
Standard #: 22VAC40-185-130-B
Description: Based on record review, the center did not obtain updated immunizations once every six months for children under the age or two years.
Evidence: Child #3 (21 months) most recent documented immunizations are from when the child was 12 months old.

Plan of Correction: I'll send the parents an email today to request updated immunizations.

Standard #: 22VAC40-185-450-A
Description: Based on observation, cots used by children other than infants did not have linens consisting of a top cover and bottom cover.
Evidence:
1. 12 out of 12 children napping in the Preschool classroom did not have bottom linens on their cots.
2. 10 out of 12 children napping in the Pre-Kindergarten classroom did not have bottom linens on their cots.
3. 13 out of 15 children napping in the Discovery Preschool classroom did not have bottom linens on their cots.

Plan of Correction: We've ordered new bottom sheets.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review and interview, the center did not obtain an out-of-state central registry (CPS) background check for one employee who lived outside of the state of Virginia within the last 5 years.
Evidence: Staff #2 (date of hire: 2/20/2018) sworn disclosure statement states that the employee lived in the state of Florida within the last 5 years. Staff #2 file does not contain documentation that the results of a central registry background check have been obtained. Center management has stated that they sent the background check out but doesn't recall receiving results.

Plan of Correction: I will call to follow-up on this background check. If they can't confirm that they received my request we will send out another background check.
