Comments:
Thank you for your assistance during this unannounced renewal inspection. This on-site visit was conducted on June 6, 2019 from 1:00 p.m. through 4:30 p.m. Staff records were reviewed at the YMCA offices on June 10, 2019. Five children's records and nine staff records were reviewed and updated. The medication and medication paperwork was reviewed for two children. Areas of non-compliance may be found on the violation notice. Areas observed: pool arrival, pool time, departure, and group activity in the gymnasium. Interactions between the children and staff were observed as appropriate and the children seem comfortable with the daily routine and schedule. Standard #: 22VAC40-185-40-D
Description: Based on observation the center failed to have the license posted in the facility. Evidence: 1. There was no documentation of the license being posted in a conspicuous public place. 2. Staff confirmed the license was not posted.

Plan of Correction: The license will be posted for parents to view.

Standard #: 22VAC40-185-510-G
Description: Based on medication review the center failed to have medication for one child in a prescription bottle with pharmacy label attached. Evidence: 1. The medication and medication paperwork was reviewed for two children. 2. The medication for Child #6 was not in the original pharmacy bottle with the prescription label attached. 3. The site director confirmed the medication was not in the original bottle with the pharmacy label attached.

Plan of Correction: The medication was returned to the parent at pickup. The director asked the parent to furnish medication in the original pharmacy bottle with the label attached.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation the center failed to post the most recent violation notice. Evidence: 1. There was no evidence of the most recent violation notice being posted. 2. Staff confirmed the most recent violation notice was not posted.

Plan of Correction: The most recent violation will be posted at the facility.
