Comments:
A Subsidy Health and Safety inspection was conducted on June 6, 2019 and June 10, 2019 to review supplemental health and safety requirements. This on-site visit was conducted on from 1:00 p.m. through 4:30 p.m. Staff records were reviewed at the YMCA offices on June 10, 2019. Five children's records and nine staff records were reviewed and updated. The medication and medication paperwork was reviewed for two children. Areas of non-compliance may be found on the violation notice. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Areas observed: pool arrival, pool time, departure, and group activity in the gymnasium. Interactions between the children and staff were observed as appropriate and the children seem comfortable with the daily routine and schedule. Description: Based on record review the center failed to have documentation of one Staff obtaining the required Virginia Preservice training by January 16, 2018. Evidence: Based on record review, Staff #5 did not have documentation of completing the Virginia Preservice Training for Child Care Staff before January 16, 2018. 2. The director confirmed she could not find the documentation for Staff #5 completing the Preservice training.

Plan of Correction: I will check with Staff #5 to see if he can produce documentation of completing the preservice training. If he is unable he will take the training and it will be documented in his file.
