Comments:
Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on July 18, 2019 from 10:30. a.m. through 1:00 p.m. There were twenty-four children in the care of six staff. Five children's records were reviewed. Staff records were reviewed on July 8, 2019 at the YMCA's office. The medication and medication paper work was reviewed for three children. Areas of non-compliance may be found on the violation notice. Areas observed were: free choice, indoor games, hand washing/bathroom procedures, craft activity, transition, and lunch. Interactions between the children and staff were observed as appropriate and the children seem comfortable with their daily routine and schedule. Standard #: 22VAC40-185-510-A
Description: Based on review of three medications, the center failed to ensure parent authorization was obtained for one emergency inhaler. Evidence: 1. Three medications were reviewed. 2. Child #1 had a Pro- Air inhaler but did not have documentation of parent authorization. 3. The site director confirmed they had not obtained parental authorization for Pro-Air

Plan of Correction: Medical authorization will be obtained for Child #1's Pro-Air inhaler.

Standard #: 22VAC40-185-550-C
Description: Based on observation the center failed to have an emergency evacuation and shelter in place map posted. Evidence: 1. The center was toured and there was not evidence of an emergency evacuation and shelter in place map posted. 2. The site director confirmed the emergency evacuation map was not posted.

Plan of Correction: An evacuation map with be posted at all entrances used by children.

Standard #: 22VAC40-185-550-F
Description: Based on observation, the center failed to post the emergency numbers and poison control. Evidence: 1. The center was toured and the there was no evidence of posted emergency phone numbers. 2. The site director confirmed emergency numbers were not posted.

Plan of Correction: Emergency numbers will be posted immediately.

Standard #: 22VAC40-80-120-E-1
Description: Based on observation, the center failed to post the most recent inspection findings. Evidence: 1. A tour of the center did not reveal the most recent inspection findings posted. 2. The site director confirmed the most recent inspection findings were not posted.

Plan of Correction: The most recent inspection findings will be posted.
