9:50am-12:15pm Conducted an unannounced religious exempt program. Observed 44 children + 6 direct-care staff today. Ratios and supervision were in compliance. Children were observed having story time, eating snack, and having active outdoor play on the playground. Staff were actively engaged with the activities and supervision of children. The facility was found to be very clean, safeguarded appropriately, and well supplied with toys and equipment for the children. In addition to classrooms, there is a multi-purpose room and an indoor climbing/sliding play room. Recordkeeping was found to be organized. Background check and safety non-compliances found.

Standard #: 63.2(17)-1716-B-6 Description: Based on observations made, it was determined that resilient surfacing on the playground was less than 6" in depth. Evidence: The wood mulch in some of the fall zones was less than 6" in depth. The area around the smaller rock wall was the lowest. Plan of Correction: A request for more wood mulch was submitted last week.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on records reviewed, it was determined that 1 of 7 staff records did not include an out-of-state child abuse and neglect search (CPS). Evidence: Staff #5 - According to the sworn statement on-file, the staff person has lived in multiple states in the prior 5 years. Management stated that they were not aware of the requirement to complete out-of-state CPS checks. Plan of Correction: An application for each out-of-state CPS check will be submitted in the next 10 days.
