63.2(17) License & Registration Procedures

Comments:
An unannounced monitoring inspection was conducted on June 18, 2019 from approximately 12:20 pm-2:30pm. There were no children present during the inspection. The center has a total capacity of 24 children. One open classroom, two rest room areas and an outdoor playground were inspected. The outdoor playground equipment is not currently being utilized. The center does not offer transportation or administer medication. Five staff records were reviewed.

Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on record review, in three of five staff records reviewed, the center did not ensure that staff have a sworn statement before employment and a results of a search of the Central Registry within 30 days of employment. Evidence: As confirmed by the Director, there was no documentation of sworn statement and or Central Registry results for the following staff members: 1. There was not a sworn statement or affirmation for Staff #3 (date of hire 9/2017) and Staff #4 (date of hire 9/2018). 2. There were no Central Registry results for Staff #4 (date of hire 9/2018).

Plan of Correction: Director will get all new sworn statements and Central Registry forms completed for all staff so that everyone is on the same schedule.

Standard #: 63.2(17)-1716-A
Description: Based on review, the center did not ensure that all required information was disclosed in writing to the parents or guardians of the children in the center. Evidence: The Director confirmed that the qualifications of the personnel had not been disclosed in writing to the parents or guardians of the children in the center.

Plan of Correction: We are updating the handbook this summer and will add that information.

Standard #: 63.2(17)-1720.1-A
Description: Based on record review, in of five staff records reviewed, the center did not ensure that staff shall undergo a background check prior to employment and every five years thereafter. Evidence: The Director confirmed that the Central Registry results for Staff #1 (date of hire 2006) and Staff #2 (date of hire 9/2012) were more than five years old and were dated 7/17/13.

Plan of Correction: Director has emailed the church secretary to print out the new forms. We will get them mailed off within the next two days.
