Comments:
An unannounced complaint inspection was conducted on April 18, 2019, regarding supervision . Interviews were held with staff, documentation was reviewed and an inspection of the facility was conducted. There was not a preponderance of evidence to support the allegation for supervision, therefore the complaint has been determined to be NOT VALID. Areas of non-compliance are identified on the Violation Notice.

Description: Based on record review and staff interview, the center did not ensure that ratios were maintained for children from birth to two years: one staff member for every four children. Evidence: During the inspection conducted on April 18, 2019, Staff #1 and Staff #2 confirmed class #1 and class #2 were combined on April 12, 2019. The attendance record (Exhibit 1) for April 12, 2019 confirmed class #1 and class #2 combined there was 7 children in the class. Staff #1 confirmed during her hours she worked between 9am-5pm and she worked alone on Friday April 12, 2019. The ratio was 1 staff to 7 children between 9am-5pm. Staff #2 stated she did work in the classroom with staff #1 from 10-11:30am. The children ages ranged from 21months to 2 years. For children from birth to two years: one staff member for every four children.

Plan of Correction: There are corrections we are putting into place as a result of the non-compliance we received for having a 22 month old in class with 2 year old causing the class to be out of ratio. 1. We will know an maintain state standards for both religiously exempt and state licensing policy. 2. We will make sure that the ratio is maintained even in cases of emergency or unforeseen circumstances by having a substitute come in or moving children to another classroom to maintain ratio. I as the director will ensure that there is not a case of teacher/child ratios not in compliance with state licensing policy.
