The Licensing Inspector for Longwood Avenue Baptist Church conducted an unannounced RECDC Monitor Inspection on Wednesday, July 17, 2019. Licensing Inspector arrived & started inspection at 11:15 am and concluded at 1:30 pm. Upon arrival, Licensing Inspector observed that there were a total of twenty-nine (29) children ages 2-4 years old and five (5) staff members present. The Licensing Inspector reviewed information for 5 enrolled children & information for 5 staff members. Areas that were inspected were found compliant with Religious Exemption requirements. There were no violations found during today?s inspection. LI Anderson conducted an exit interview along with reviewing the Risk Assessment and the areas of non-compliance with the center?s Administrator on July 17, 2019. Standard #: 22VAC40-191-40-C-2 Description: Based on review of staff information, the Center failed to ensure each employee, volunteer or any other person who is expected to be alone with one or more children enrolled in a religious exempt child day center has completed Sworn Disclosure Background Check before employment/commencement of service and Central Registry Background Check within 30 days of employment/commencement of service. EVIDENCE: Licensing Inspector reviewed information for five (5) staff members. Information for Staff #2 and Staff #5, did not include evidence of completed Sworn Disclosure. Information for Staff #1, #2, #4 and #5 did not include evidence of a completed Central Registry (CPS) Background Check. Plan of Correction: The provider must send documentation to the LI that the background checks have been requested no later than 10 days following this notification date 7/17/2019
Standard #: 63.2(17)-1716-A-6 Description: Based on review of center information, the Center failed to provide a written statement regarding qualifications of personnel, physical facilities, health requirements for the staff and public liability insurance to the parents/guardians of the children in the center and made available to the general public. EVIDENCE: A written description of the center's personnel qualifications, physical facilities, information regarding the center's health requirements for staff and information regarding the center's public liability insurance were not included in the parent handbook nor posted. Plan of Correction: Center will update parent handbook information to include required information
Standard #: 63.2(17)-1716-B-4 Description: Based on inspection & discussion with the Center Director, the Center failed to ensure all children in the center are in compliance with the regulations regarding the immunization of children against certain diseases. EVIDENCE: Licensing Inspector reviewed information for five (5) enrolled children. 2 out of the 5 children (Child #2 and #4) did not have information regarding immunizations against certain diseases. Plan of Correction: Center will have parents submit documentation of immunizations.
Standard #: 63.2(17)-1720.1-A Description: Based on review of staff/volunteer information, the Center failed to ensure all employees and volunteers complete Background Checks (Child Protective Services; Fingerprint & Sworn Disclosure) every five years. EVIDENCE: Licensing Inspector reviewed information for 5 staff members. Staff #3 did not undergo a Sworn Disclosure Background Check every 5 years. Sworn Disclosure was last completed on 7/24/2000 Plan of Correction: The Center must send documentation to the LI that the background checks have been requested no later than 10 days following this notification date 7/17/2019
