Comments:
An unannounced monitoring inspection was conducted beginning at 9:15 a.m. and ending at 12:30 p.m. There were 66 children in care ranging in age from 2 to 5 years of age under the care and supervision of 14 staff. Children were observed during a performance of The Nutcracker and preparing for Chapel. Center documents and 17 staff records were reviewed.

Description: Based on review of staff records and interview with staff, it was determined that the center did not obtain sworn statement or affirmations by the first day of employment and a central registry check within 30 days of employment. Evidence: Staff #2, hired 8/17/2011, sworn statement was dated 9/5/19. Staff #3, hired 8/2018, sworn statement dated 9/5/19.

Plan of Correction: In the future, all sworn statements will be completed by the first day of employment. The central registry background check delay was a clerical error.

Description: Based on review of documentation, it was determined that the center did not have documentation of annual health reports. Evidence: Staff #1, Staff #2, Staff #3, Staff #4, Staff #5, Staff #6, Staff #8, Staff #9, Staff #11, Staff #12, Staff #16 did not have documentation of annual health report.

Plan of Correction: Documentation has been misfiled and will be located and filed as soon as possible.

Standard #: 63.2(17)-1721.1-B-2
Description: Based on review of staff records, it was determined that the center did not document results of national fingerprint background checks as required. Evidence: Staff #12, hired 8/2017, and Staff #17, hired 8/ 2017, did not have documentation of national fingerprint background checks.

Plan of Correction: Staff underwent fingerprinting with the state police. Both staff will obtain national fingerprint background checks as soon as possible.

Standard #: 63.2(17)-1721.1-B-3
Description: Based on review of documentation and interview with staff, it was determined that center did not request out of state central registry background check for staff that resided outside of Virginia in the last five years. Evidence: Staff #2, hired 2017, indicated out of state residence in South Carolina on the sworn statement dated 9/5/2019 and there was no documentation of a request of a South Carolina central registry background check.

Plan of Correction: A South Carolina central registry check will be requested as soon as possible.
