Comments:
An unannounced monitoring inspection was conducted today between 1:00pm and 3:00pm. There were 8 children (21 points) present with two caregivers. The entire physical plant and outdoor space was inspected. Caregiver's/children's/household members records, menus, attendance, emergency supplies and emergency procedures were reviewed. The children were observed eating lunch, handwashing, playing outside, diaper changing and preparing for naptime. Lunch consisted of noodles, chicken, mixed vegetables, apples and milk. Standard #: 22VAC40-111-60-A
Description: Based on children's record review, the provider failed to maintain an up-to-date record at the family day home for each enrolled child. Evidence: 1) Child#1's file did not have documentation of immunizations on the first day of attendance or a physical by the 30th day of attendance. 2) Child#2's file did not have an authorization to pick up form or documentation of a physical by the 30th day of attendance. 3) Child #5's file did not have documentation of a physician's phone number or a physical by the 30th day of attendance.

Plan of Correction: I will get forms from parents and will put them in the child's file.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider failed to ensure that areas and furnishings of the family day home inside and outside will be maintained in a clean, safe and operable condition. Evidence: The infant swing in the outside play area is broken and no longer has the safety strap attached.

Plan of Correction: I will remove the infant swing.

Standard #: 22VAC40-111-280
Description: Based on observation, the provider failed to ensure that potentially poisonous materials will be stored away from areas accessible to children. Evidence: There is a can of Wasp and Hornet Spray in the outside play area accessible to children.

Plan of Correction: Wasp spray was removed during inspection.
