Comments:
An unannounced monitoring inspection was completed today. The physical plant, 4 caregiver/household member files, 5 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 2:10-3:25pm. There were 10 children (plus the Provider's grandson), totaling 33 points, under the supervision of 1-3* caregivers. *See Violation Notice. Today, the children rested during naptime, ate afternoon snack, enjoyed free play and rested during naptime. The interactions between the children and caregivers were seen to be positive and constructive in nature. The children were served spaghetti with cheese in a spinach sauce, melon and milk for lunch. The Provider is not certified in MAT (medication administration training) and there are no children with any medication needs. Standard #: 22VAC40-111-570-A
Description: Based on file review, interview and observations, the Provider did not ensure that caregivers only care for children totaling up to 16 points by using the point system based on the children's age. Evidence 1. When the LI arrived to the FDH, it was 2:10pm. After no one answered the door, the LI called the Provider at 2:14pm. The Provider informed the LI that she had run out and would be back shortly. The Provider arrived back to the home at 2:20pm. Upon entering the FDH it was determined that while the Provider was gone, there were 10 children in care, totaling 32 points under the supervision of 1 caregiver. Once the Provider returned with her grandson, the points went up to 33 points, which required 3 caregivers present and there were only 2 at this time. At 3pm, a household member/caregiver arrived to the FDH. At this time, the point system requirements were met.

Plan of Correction: The Provider will not leave the FDH when she is needed to maintain the point system requriements.

Description: Based on observations, the findings of the most recent inspection of the facility were not posted. Evidence 1. The Violation Notice from the inspection on 10/25/18 was not posted. The Provider had mistakenly posted the Inspection Summary (which does not include the findings).
