Comments:
An unannounced monitoring inspection was conducted today from 10:45am-12:30pm. There were 10 children (27 points) directly supervised by 2 staff. The home, 2 staff records, 4 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and building with blocks. There was a supply of books, toys, and materials for the children. The home was clean and organized. Violations:
Standard #: 22VAC40-111-760-A-1
Description: Based on observation, the provider did not maintain all required items in the first aid kit. Evidence: The first aid kit is missing one triangular bandage.

Plan of Correction: We will purchase another triangular bandage.

Standard #: 22VAC40-111-180-A
Description: Based on observation, the provider did not obtain for each caregiver a current Report of Tuberculosis screening form every two years from the date of the most recent screening. Evidence: Household member 1 has a tuberculosis test conducted on 3/4/17 and has not been updated.

Plan of Correction: I will make sure to sent them to get an updated screening.

Standard #: 22VAC40-111-510-A
Description: Based on observation, a child was not provided with an individual crib or rest mat for resting or napping. Evidence: Child 1 was observed sitting on a carpeted floor with a comforter underneath them after waking up from a nap.

Plan of Correction: I will put a mat to sleep immediately.

Standard #: 22VAC40-111-530-F
Description: Based on observation, the provider used a comforted with an infant. Evidence: Child 1 was observed with soft comforter underneath them and had just woke up from a nap. (Child 1, DOB: 5/14/18)

Plan of Correction: I will make sure the child has a thin blanket.

Standard #: 63.2-1720.1-B-2
Description: Based on record review, the provider did not ensure al staff submitted to fingerprinting. Evidence: Staff 1 (Date of Hire: 12/2017) does not have a completed fingerprinting criminal history record check.

Plan of Correction: I will make an appointment for the staff member to complete fingerprinting.
