Technical Assistance:
The next anticipated inspection will be the provider?s renewal inspection. Provider was reminded to have all renewal application documents completed and sent into licensing office 45 days prior to expiration for processing. The renewal application is available on the VDSS website.

Comments:
Today an unannounced Monitoring inspection took place between 11:35am and 1:20pm. There were 11 children present with the provider and 2 assistants for a total of 25 points within the supervision guidelines. At the start of the inspection the preschool children were observed playing outside; the infants and toddlers were sitting in their high chairs waiting for lunch to be served. The children were observed washing their hands and eating lunch (chicken and rice soup w/ green beans, cucumber, mango and water). The provider is not currently trained to administer medication and no medication is on site. Positive interactions between the caregivers and children were observed. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain all required information. Evidence: Child #1 file did not contain an address for their second emergency contact.

Plan of Correction: I have already asked the parents to prove the address. I'll ask them again today.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis (TB) Screening form every two years from the date of the first screening. Evidence: Caregiver #2's most recent TB screening is dated 5/2/2017. Caregiver #3's most recent TB screening is dated 5/9/2017.

Plan of Correction: We'll get it done this week.
