Comments:
An unannounced monitoring inspection was conducted on 05/20/2019. There were 4 children, ages 23 months-3 year, totaling 9 points and one provider present during the inspection. The inspector reviewed 5 children's records, one provider record and one household member's record during the inspection. The provider stated that there is no medication being administered. The children were observed in the following activities: watching television, in free choice activities, music and movement and during nap time. The inspector reviewed the required information for parents, emergency preparedness plans and the emergency supplies during the inspection. The inspector discussed hiring a new assistant and background check requirements. The inspector arrived for the inspection at 11:25 a.m. and departed at 1:30 p.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record review and discussion, the provider failed to ensure that each child's record shall contain all of the elements as required by the standards. Evidence: 1. The record for Child #1 did not contain the following information: the name, office address and telephone number of the child's physician (there was not a complete address for the physician), results of the health examination for the child as required by 22VAC40-111-100 (there was not a physical in the record) and the name and addresses of previously attended child day care. 2. The record for Child #3 did not contain the following information: name, address and telephone number for each custodial parent's place of employment (both parents had places of employment listed but no telephone number of the place of employment was provided), parent's signed acknowledgement of the receipt of the information required by 22VAC40-111-70, the first date of attendance, and the names and addresses of previously attended child day care. 3. The record for Child #5 did not contain the following information: proof of the child's age and identity and the results of the health examination for the child as required by 22VAC40-111-100.

Plan of Correction: The provider will ask the parents for the information at pickup today.

Standard #: 22VAC40-111-70-A
Description: Based on review of records and discussion, the provider failed to ensure that before the child's first day of attendance, parents shall be provided in writing with all of the information as required by the standards. Evidence: The Parent Information Model Form for Child #2 and Child #4 did not contain information regarding the general daily schedule that is appropriate for the age of the child.

Plan of Correction: The information will be added this evening.

Standard #: 22VAC40-111-180-A
Description: Based on record review and discussion, the provider failed to obtain, for each caregiver and adult household member, a tuberculosis screening every two years from the date of the first screening. Evidence: 1. The record for Provider #1 had a tuberculosis screening dated 7/18/2016. There was no documentation of a 2018 screening. 2. The record for Household Member #1 had a tuberculosis screening dated 7/18/2016. There was no documentation of a 2018 screening.

Plan of Correction: An appointment is scheduled for July but will call this evening to see if an appointment can be scheduled sooner.

Standard #: 22VAC40-111-280
Description: Based on observations and discussion, the provider failed to ensure that potentially poisonous substances, materials and supplies such as but not limited to, cleaning agents, disinfectants, plant care chemicals, pesticides, and petroleum distillates shall be stored away from food in areas inaccessible to children. Evidence: There was a can of Raid on top of the refrigerator beside of a package of hotdog buns. There were also several boxes of cereal on top of the refrigerator.

Plan of Correction: The provider will move the Raid.
