Comments:
An unannounced monitoring inspection was conducted today with the provider, who was present with 4 day care children for a total of 8 points. T he children were engaged in free play during the inspection. Three caregiver files and 5 children's files were reviewed. There were no medication on site and the provider reported that there have been no injuries/accidents. Arrival time: 10:40 am Departure time: 12: 45 pm

Violations:
Standard #: 22VAC40-111-830-C
Description: Based on interview, it was determined that documentation of the evacuation drills and shelter in place drills were not maintained.

Plan of Correction: I will continue to locate the documentation.

Standard #: 22VAC40-111-60-B
Description: Based on a review of five children's files, it was determined that two children did not have emergency contacts on file. Evidence: Child A and B enrolled on 6/26/18 and neither child had emergency contact information on file.

Plan of Correction: The emergency contact information was obtained during the inspection.
