Technical Assistance:
Reminder that each caregiver must complete 16 training hours per year.

Comments:
An unannounced monitoring inspection was conducted today between 9:00am and 12:00pm. There were 6 children (16 points) present with 2 caregivers. The children were observed playing with wooden puzzles, riding toys, playing outside and an infant was observed playing with caregiver, bottle feeding and nap. The physical plant, 2 caregivers files, 7 children's files and 2 household members files, evacuation log, attendance log, emergency supplies, menu's and cribs were inspected. Violations:
Standard #: 22VAC40-111-710
Description: Based on record review and interview with provider, the provider failed to obtain written authorization from a parent to administer medication. Evidence: Child # 7 has emergency medication at the family day home for a diagnosed condition. The provider does not have written authorization from the parent to administer.

Plan of Correction: I will not administer medication.

Standard #: 22VAC40-111-960-E
Description: Based on observation, the family day home failed to ensure that bottles are labeled with the child's full name and the date. Evidence: Observed bottles for Child # 3 was not labeled with child's name and date.

Plan of Correction: I will remind parents to label and date bottles.

Standard #: 22VAC40-111-100-A
Description: Based on a review of seven children's records, 4 of 6 records did not have documentation of a physical within 30 days of attendance. Evidence: Child #2, #3, #4 and #6's files did not have documentation of a physical within 30 days of the first day of attendance.

Plan of Correction: I will get proof of physicals from parents.

Standard #: 22VAC40-111-60-A
Description: Based on review of seven children's records, 2 of 7 records were incomplete. Evidence: Child #5 and #7's files do not have documentation of the Names of persons other than the custodial parents who are authorized to pick up the child.

Plan of Correction: I will get from parents.

Standard #: 22VAC40-111-90-A
Description: Based on a review of seven children's files, 4 of 7 children did not have proof of immunization by the first day of attendance. Evidence: Child #2, #4, # 5 and #6's files do not have documentation of immunizations by the first day of attendance.

Plan of Correction: I will get immunization for first day from parents

Standard #: 22VAC40-111-220-A
Description: Based on caregiver record review, the provider failed to ensure that prescription medication will be administered by a caregiver who has satisfactorily completed a training program for the purpose or developed or approved by the Board of Nursing and taught be a registered nurse, licensed practical nurse, doctor of medicine or osteopathic medicine or pharmacist or is licensed by the Commonwealth of Virginia to administer medication. Evidence: Caregiver #1 or #2 are not Medication Administration Training Certified.

Plan of Correction: I will return medication to parents.

Standard #: 22VAC40-111-280
Description: Based on observation, the family day home failed to ensure that potentially poisonous substances will be stored in areas inaccessible to children. Evidence: Cleaning wipes were in an area accessible to children.

Plan of Correction: Removed during inspection.
