Comments:
The licensing inspector conducted an unannounced renewal inspection on Tuesday, August 6, 2019 between 10:00 am to 11:50 am to determine the provider's compliance with the licensing standards. During this inspection, there were no children in care, however reportedly there are six child enrolled at family day home. The family day home designated childcare areas were inspected. The provider's record and four children's record were reviewed during this inspection. The family day home's first aid kit and emergency supplies were inspected. The Department has not yet received a complete renewal application and annual fee from the center. The Department must receive a complete renewal application and fee prior to expiration of the license on September 6, 2019 or the Department cannot renew the license. Standard #: 22VAC40-111-100-A
Description: Based on review of children's and interview with provider, the provider did not ensure to obtain a physical examination for all children prior to child's attendance or within 30 days after the first day of attendance. Evidence: In Child's #4's record (start date 09/15/14), there was no physical examination documentation found. Interview with provider confirmed a physical documentation was not available during this inspection.

Plan of Correction: The provider will obtain documentation for child's physical immediately. Provider will ensure to include physical documentation in child's record.
