Technical Assistance:
Reminder: Two shelter practice drills to be completed by the end of 2019 Reviewed requirement/procedure for request of an out-of-state central registry check

Comments:
An unannounced monitoring inspection was conducted on 08/02/2019 from 10:55am - 12:35pm. At the time of entrance there were nine children in care with two caregivers. The ages of the children counted for a total of 20 points. Children were observed reading books and engaged in music and large movement activities. Lunch was additionally observed. Records reviewed for two caregivers and nine children. Two violations were observed in the areas of home maintenance and background checks. These violations are listed on the violation notice and were reviewed with the provider at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider failed to ensure that outside areas of the family day home shall be maintained in a safe condition. Evidence: One paver in the backyard play area has a broken area that is a trip hazard.

Plan of Correction: The paver will be replaced on Thursday, August 8th. In the meantime, this area is covered and off limits to everyone.

Standard #: 63.2-1720.1-B-3
Description: Based on record review and interview, the provider failed to obtain a copy of the results of a search of the central registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him. Evidence: 1. There was not a central registry check on file for staff 1 who indicated on her sworn statement or affirmation that she has reside in the State of Georgia within the past five years. 2. The provider confirmed that a central registry check from the State of Georgia had not been requested.

Plan of Correction: An out-of-state central registry check will be requested.
